When a patient moves between clinics, hospitals and cities, their history usually stays behind. Allergies get missed, tests get repeated, and conditions slip through the cracks. Sagma Health exists to close that gap. We're working with providers to build a health record that travels with the patient, so care is safer and more continuous across every facility.
